Crisis Companion™ provides practical, family-focused emergency identification tools for your vehicle. Each laminated 5×8 bilingual card lists vital contacts, allergies, and medical information so first responders can access what matters most when every second counts.
How do I personalize my Emergency Contact Card?
After purchase, you’ll receive a secure, encrypted JotForm link in your confirmation email. You’ll complete one form per card purchased, upload your photo if desired, and submit. Your information never appears publicly and is not stored online.
What kind of services does Crisis Companion™ offer?
Crisis Companion™ provides physical family safety products; not mental-health services or crisis-management software.
Is my personal information safe?
Yes. All data is submitted through a secure, encrypted form (SSL + end-to-end encryption). Crisis Companion™ never stores or views your private details only you hold the printed, physical copy once your card is produced.
Where should I keep my emergency contact card?
We recommend storing your card inside the included privacy envelope and placing it in your vehicle’s glovebox, door pocket, or emergency binder. Keep additional copies in purses, backpacks, or travel kits for quick access.
What makes Crisis Companion™ different?

Is Crisis Companion™ a mental-health service?
No. Crisis Companion™ provides physical safety products such as emergency contact cards and seatbelt covers. We do not offer therapy or crisis-management software.
Can I order for my whole family?
Absolutely. Choose from our 2-pack or 4-pack bundles, or shop family kits that include seat belt covers and money clips. Each card comes with its own encrypted form link so every member’s details remain private.
When will I receive my order?
Processing begins after your encrypted form submission. Most orders ship within 3–5 business days after forms are received. You’ll receive an email update when your order is complete and ready to ship.
